Appointments

An intake screening form will be sent to you; completing intake screening forms does not indicate a provider-patient relationship. Forms must be returned, reviewed by provider and you will be contacted to schedule appointment  within 2-3 business days if approved.

After initial evaluation, if a treatment plan is agreed on; a follow up appointment will be scheduled.
